Types of SMS Communications

Our Clients: Appointment reminders, confirmation, cancellations or rescheduling notifications, updates on treatment plans, information related to upcoming meetings and notification of documents that require your attention. and any follow up care instructions.

Prospective clients: Information about available services, consultations, or information pursuant to your request. 

Job applicants: Updates about application status, interview schedules, and other employment-related communications. 

We may share your personal information in the following situations

  1. With service provider organizations from whom you may be receiving services.  If we are party to a service plan with other agencies from whom you also receive services, we may exchange information about you with those agencies.  Additionally, if you have authorized us to provide your information to a prospective service provider we will do so, but not without your permission. 
  2. With your physician(s) to secure information about your continued eligibility for services and to the degree, for which you may be eligibile for various services.
  3. With affiliated firms: While we have but one at the present time, we may share your information with related firms for the purposes of doing business with you and or to ensure that your needs are being met.   (Our billing is on occasion done by a third party with whom we have an agreement and who’s personell are covered by our background checking requirements. )
  4.  With our regulatory agencies.  At their request we will (to the extent required by law) provide your information to those agencies.  This for example can be for audit purposes in which an appropriately certified audit provider is provided copies of our service notes to compare those to our billing information.  
  5. With your consent: we may disclose your personal information for any other purpose with your consent.

Important as relates to the above:

  • Abacus Care Coordination Inc., is governed by licensure and regulation at the established by the centers for Medicaid and Medicare. We are a medical provider and the information indicated avove is provided within the context of our providing services to you and for need without which we would be unable to continue to provide you with care coordination services. 

  • Ourside of our need to do so in any of the 5 reasons above we will not provide your information without your request.

  • Should we fail to provide the above language above we will not have the freedom to contact you via SMS messages pursuant to your preferences previously established in conversation with us.  Understand that we are held to a much higher standard and that your information is released only to those organizations for whom you have given us permission and for whom your releases of information apply and are intended.  

Comments

When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.

An anonymized string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. The Gravatar service privacy policy is available here: https://automattic.com/privacy/. After approval of your comment, your profile picture is visible to the public in the context of your comment.

Media

If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.

Communications and Privacy information

We will communicate with you via any means you provide or authorize us to use.  During the course of our communications with you we will ask for your preferred means of reaching you.   If at any time you wish that we suspend communications via a specific means please inform us using that means of communication and by stating please do not communicate with me on this [platform, phone number, text number, email etc.] and we will send you a confirming communication using that means and then remove it from our system.  We will only provide sensitive information to you on confirmed means of communication.  If your contact information should change please keep us updated and we will gladly adjust our communications to you accordingly.  We will collect communication preferences via our website, via phone or any other means so to serve you better.   With your permission, we may communicate via: Email, Text, Phone calls etc. We will not provide information regarding your services to any third party without your permission.  Such permission can be provided verbally in some cases or in writing in any form (usually with a Release of Information form) as long as we can confirm that the authorization is coming from you.  Your contact information will not be made available to other persons with the exception of state agencies with whom we collaborate to provide you with your services.  You will not recieve marketing of any nature from Abacus but you may receive information of general interest (regarding waiver services) in the form of an occasional newsletter or something of that nature.  Additionally, we are required to recertify periodically, part of that process is to collect survey information on how we are performing providing your services.  These surveys are sent out annually (and not to everybody – we use a measured cross section of those we serve and work with) and are optional, if you would prefer not to participate you may ignore the survey.  Beyond this we desire to protect your privacy at every opportunity, if there is anything specific that concerns you, please commuicate with your Care Coordinator and share your concerns accordingly.  We will make every attempt to satisfy your concerns. SMS opt-in and Phone numbers for the purpose of SMS will not be shared for marketing purposes and at no time will your health care information be shared via SMS communications

Cookies

If you leave a comment on our site you may opt-in to saving your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you leave another comment. These cookies will last for one year.

How long we retain your data

If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.

For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.  Health Care information related to your Medicaid waiver is kept on file for the required period (7 years) and is available to you on request.  If there is anything you desire please let us know and we’ll be happy to provide you with the entirety of information we have. 

What rights you have over your data

If you have an account on this site, or have left comments, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us. You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.

Other information about communications

  • SMS consent is not being shared with third parties or affiliates. Nor is any other information.  We are required by law to release certain information to the State of Alaska under certain condtions.   If you should get messages that are from any source and you question it’s validity use caution and reach out to the orginator of the message.   If you think we may be able to help you please call us immediately.  
  • If you have consented to receive text messages from Abacus Care Coordination Inc.  you may receive text messages related to your services, or other services you may be entitled to. We do not “market” via messaging or email.  You may receive an invitation to participate in a performance review of Abacus and your care coordinator we are required by our licensing body to collect answers to such a survey.  Please respond to this survey as it will improve your services and service delivery. .  Otherwise we may use text messaging to relay a question about you in an effort to provide required information to state or federal regulators.    Message and data rates may apply.  Messaging frequency may vary. Data Protection

We make every attempt to protect your data, anything we store is securely sequested behind a username and password login which requires 2 factor identification to access on-line information and is securely locked up in our office for other data and Medicaid / Waiver information.  There is little stored online in the first place and then only that information you provide to our website.  If you have any questions please feel free to reach out to me at “Bob@abacuscarecoordination.com” and I will be happy to speak with you regarding this or any other matter.

Privacy Policy updates

Changes to our privacy policy will be posted here on our website for public consumption.  As we are governed by both SMS-TCR and HIPPA regulations changes are infrequent and you will also be notified by your chosen means of communications that this has occured along with a link to our privacy policy.  If you have any questions please feel free to reach out and we will answer them accordingly.
